How can you help a severe bipolar alcoholic?

How can you help someone who's a severe bipolar alcoholic, who lies compulsively, is verbally abusive, goes from severe bouts of raging mania to severe lows of attempting suicide? He's been in all kinds of treatment: extended stays in hospitals, tried all med combinations to now nothing. He's still in therapy, but he lies to them and leaves and does what he wants - everything bad and out of control. What can be done, if anything? He's irrational, you can't speak reason to him. This has been going on for many years. Any recommendations?
